We need your help! The 2021 Central Minnesota Community Health Survey is a collaborative project between local public health agencies and CentraCare to gather information that will create an accurate picture of health and wellness across Central Minnesota.
The survey is available in three languages: English, Spanish and Somali. This electronic format is a parallel project with a mailed survey that was delivered to over 7000 households across Benton, Sherburne, and Stearns Counties. If you received the mailed copy, we would encourage you to submit the mailed copy.
